Before arriving at Sky Ranch, every camper is required to fill out an online Health History Form. This information is kept strictly confidential and will only be used by our nursing staff in case of an emergency. This year, this form can be completely filled out online at http://register.skyranch.org/parents. Please take a few minutes to complete this before your child comes to camp.

If your child is taking prescription medications, you must personally check them in with the Health Center upon arrival to camp. If someone other than the legal guardian will be bringing you child to camp please download and complete this form and bring the signed copy to check in.
